My name is David Northcutt. I'm oneof the podiatrists here at Dallas Podiatry Works. Today I want to discuss diabetic peripheralneuropathy. There are several reasons for the development of peripheral neuropathy,but diabetic peripheral neuropathy is one of the most common. Diabetic peripheral neuropathy simply meansnerve damage that is caused from having diabetes. This is not something that develops rapidly,but is a slowly worsening and progressive condition which happens over the period ofseveral years. The loss of sensation that occurs with nerve damage from diabetes makesthe patient more prone to developing open

sores or ulcers. Patients often do not know that they havea sore or wound, due to this lack of sensation. This can lead to significant complicationsincluding amputations. Diabetic peripheral neuropathy usually occurs in patients whodo not maintain their blood sugar well, however it can occur in anyone with diabetes. The symptoms of neuropathy include numbness,tingling, pain, burning in the feet which can progress up to the legs, there's oftenloss of muscle tone, loss of balance, and changes to foot structure. To determine whetheryou have diabetic peripheral neuropathy, a

history and physical will be performed. Simple,in office, noninvasive testing helps to diagnose the problem. Neurologic tests sometimes areordered. Sometimes a biopsy of the peripheral nerves in the skin may also be performed.Treatment of diabetic peripheral neuropathy begins with good control of your blood sugar.There are oral medications as well as topical medications that may reduce your symptoms.Prevention of diabetic peripheral neuropathy includes maintaining good blood sugar levels. I've eithergot high arches or I've got flat feet or I've got…� I don't know what the hell theythink, but they go, “I can't do that.� And unless you're in some extreme situationwhere you've got some real biomechanical problem, I've just never seen that. I've literally,out of the thousands and thousands of people

that I've met who talk about this at all,I've met one or two who really couldn't do it. And these are people who had, like, diabeticperipheral neuropathy or they were missing toes; even that doesn't necessarily stopyou. Or, I know people who have one leg that's an inch and a half shorter than the other;that hasn't stopped them. So the simplest thing I can say is that human beings, we don'thave very good memories. What most people don't remember is, prior to the inventionof the padded motioncontrol running shoe 45 years ago, roughly, every shoe was justlike a thin piece of rubber or leather and people all around the world for thousandsof years have been running in either that

or barefoot. And there's no reason you can'ttoo, unless again, you've got some serious, serious real thing. Flat feet and high arches,not an obstacle. Flat feet and high arches, mostly that's about…what you really needto do is do a little bit of stretching if you have high arches because they tend tobe tight, and then whether you have high or low arches, strengthen your arches. The importantthing and the fastest way to get that strength, running barefoot. Go back to the tutorial aboutwhat you need to do strengthen your arches. Running is going to be the thing that doesit the most. I had crazy flat feet my whole life. When I got out a swimming pool, my footprintlooked like a paddle. And I wish I had pictures

of that because now I get out of a pool, myfootprint looks like… a foot. Now, it's not some really high arch—arch height ismostly genetic—but I've got strong arches. I no longer need orthotics. I used to havea 500dollarayear orthotic habit. Now, I'm not telling you that that's going to happenfor you. I'm not a . I don't play one on TV. I'm not making medical claims.I'm just saying that what we see is that strength is the important thing. Running can buildthat. And there's nothing that stands in the way between where you are now and making thetransition other than being willing to be patient.
